Our goal was to re-design the patient graduation experience at Kaiser Permanente, which involves the experiences that patients go through when they are discharged from the hospital and when they are at home. After observations and interviews, we decided to tackle the problem of medication compliance as we realized that this is a huge issue in the healthcare industry. We developed a user friendly reminder system for patients and their care givers to keep track of medication and physical therapy schedules.
